He graduated from Tufts Medical School, Boston, Massachusetts and completed his internship and residency at Albert Einstein Montefiore Medical Center in New York. He is a board-certified Internist and also an experienced Emergency Physician having served as Director of Broward Medical Center emergency department for many years when he first relocated to Florida.
Dr. Hevert is on staff to Boca Raton Community Hospital and follows his patients at area rehabilitation and skilled nursing facilities. He volunteers to provide medical care such as a clinical preceptor for Florida Atlantic University Medical students. He serves as a Medical Advisor to the Louis & Anne Green Memory and Wellness Center, located at the FAU campus! He is married with three children.
Dr. Chrzan speaks fluent English, Polish and Spanish.
He has a broad medical background having worked in Occupational Medicine, and Emergency Medicine in Newark, New Jersey and New York and served as consulting Toxicologist and a member in the Fellowship Program at Hackensack University Medical Center, Hackensack, New Jersey.
Dr. Montalvan had his own Internal Medicine practice for over fifteen years in Rochelle Park, New Jersey and admitted to renowned Hackensack University Medical Center.
Dr. Montalvan relocated to Boca Raton and admits to Boca Raton Regional Hospital and the Delray Medical Center. He is an associate clinical professor at Schmidt School of Medicine at FAU. He speaks fluent English and Spanish
She and her husband relocated to South Florida in 2009 with two beautiful Golden labs. Dr. Plaxe and her husband live in Boca Raton, she is pleased to practice medicine “closer to home” having practiced initially in Boynton Beach.
